HALF DAY KINDERGARTEN REGISTRATION
MARCH 5, 2012 – March 9, 2012
9:00 a.m. to 11:00 a.m.
Eligibility: Child must have reached their fifth birthday on or before September 1, 2012.
Registration takes place at the assigned school. school assignments
Parents/Guardians do not need an appointment to register.
The eligible student does not have to be present during registration.
Full Day Kindergarten Lottery Information will be available at registration.

Needed for Registration:
1. Proof of Residency Policy 6.19 . This link will take you to the School Committee Electronic School Board site. Under the Reference Library, go to Policies & Regs. From the pull down menu select 6.0 Pupil Policies to find Policy 6.19.
2. Child’s Original Birth Certificate
*Municipal issued copy with a raised stamp or seal. Hospital documentation is not accepted. For Kindergarten the Birth Certificate must show that your child will be 5 years of age on or before September 1.
3. Most current Physical Exam with proof of Lead Screening and Immunizations
*Most physicians will provide a computer generated school health record which includes all immunizations and lead screening after a physical. The following vaccination series is required for Kindergarten and are due prior to entrance in September: HepatitisB, Varicella (Chicken Pox), DTaP or DTP, Polio, and MMR vaccinations.

Information for Homeless Families
Children With Special Needs:
Parents having a child ages three or four, or otherwise entering school, who believe that their child has special needs (learning, emotional, health, vision, hearing, physical, etc.) which may interfere with future educational progress should contact Mt. Pleasant Pre-School and discuss such needs with the Director or designee at 508-830-4347.
Students Enrolled in Private Kindergarten, who are anticipating entering first grade in the Plymouth Public Schools in September, should also register at this time.
