Student Registration

Welcome to the Plymouth Public Schools! This page will help you determine where your child will attend school in our district and what information you will need to bring for enrollment.

The following document will help you determine which school your child will attend. It contains the school assignments for elementary, middle, and high school based upon your street address.

Kindergarten Enrollment

KINDERGARTEN REGISTRATION 2017-2018

OPEN REGISTRATION DATES

February 27 – March 3, 2017

9:00 – 11:00am


Kindergarten Information Booklet


  • The Town of Plymouth now provides district-wide full-day kindergarten.

  • To enroll a child must have reached their 5th birthday on or before September 1, 2017.

  • Registration takes place at the child's assigned elementary school.

  • Parents/Guardians do not need an appointment to register.

  • Eligible students do not need to be present during registration.

  • All required documentation must be provided to complete registration.

Required Outside Documentation

You will need to provide the documentation listed below to register your child for Kindergarten:

1. Proof of Residency
 In order to attend Plymouth Public Schools, a student must reside in the Town of Plymouth. Families registering must demonstrate Plymouth residency as outlined in the Plymouth Public Schools Proof of Residency Guidelines. If parent/guardian cannot produce the documentation to prove residency a notarized Residency Affidavit is required.
2.Child’s Original Birth Certificate
 Municipal issued copy with a raised stamp or seal. Hospital documentation is not accepted.
3.Most current Physical Exam with proof of Lead Screening and Immunizations
 Most physicians will provide a computer generated school health record which includes all immunizations and lead screening after a physical. The following vaccination series is required for Kindergarten and are due prior to entrance in September: HepatitisB, Varicella (Chicken Pox), DTaP or DTP, Polio, and MMR vaccinations

Grades 1-12 Enrollment

You will need the following forms to register your child in our district schools (links to actual forms are underlined):

 

1.  Proof of Residency

- Residency Affidavit (if necessary)


2. Child’s Original Birth Certificate

Summer Enrollment

The following lists the dates/times during the summer when you can enroll your child in our schools for the fall. Please click on the Street Listings link above to determine which school your child should attend. Also, please click on the Grades 1-12 or Kindergarten Enrollment menu to see a list of necessary forms and paperwork you will need to enroll your child in our schools. It is always advisable to call the school prior to making the trip to ensure they are open and have enrollment personnel on staff that day.

 

Elementary Schools

All Elementary school main offices are open two weeks prior to the start of the school year.  We have an elementary enrollment secretary at Plymouth Community Intermediate School on the following dates from 9:00 AM to 12:30 PM:

July 11, 12, 18, 19, 25, 26, August 1, 2, 8, 9

Secondary Schools

Both middle schools, PCIS and PSMS, and both high schools, PNHS and PSHS, are open all summer during regular hours, M-F, 8:00 AM - 1:00 PM. You may need to schedule a separate enrollment appointment at the high schools for scheduling purposes.

 

Student Withdrawal

In Town Out-of-District Requests

IN-TOWN OUT-OF-DISTRICT PLACEMENT REQUESTS

Please refer to School Committee Policy 9.20 In-Town Out-of-District School Placement when considering a request to transfer or enroll your elementary or middle school child in a Plymouth school outside of the boundaries of the school they are districted to attend.  Requests for in-town out-of-district placement of high school students are not being accepted at this time.
