🚨 Important Info: 2026–2027 Back-to-School Forms 📋

Back-to-school forms will be sent via email to each student’s primary contact by the end of the first week of school.

1. Digital Forms (via Informed K12)
You will receive separate emails for each child (with your child’s name and form title in the subject line) from forms@informedk12.com for:
- Emergency Health Information
- Student Handbook (2026–2027)
- Liability Release (if not already signed for 2026–2027)
- Medicaid Parental Authorization (if not already on file)

2. Contact Information Updates (via Aspen)
You will also receive an email from aspen-sysadmin@myfollett.com with instructions on updating your student's contact details in the Aspen Parent Portal.

Tips & Next Steps:
- MARK AS SAFE: Add forms@informedk12.com and aspen-sysadmin@myfollett.com to your safe senders list to prevent emails from going to spam.
- Paper Copies: Paper forms are available upon request directly from your child’s school.
